Transformational Breath® Events Waiver and Release

I agree to participate in Transformational Breath® group or private events at blosm studio and give permission to be facilitated. I understand that the information and breath sessions offered are designed to enhance quality of life and support holistic well-being, and are not intended to constitute medical advice or any substitution for medical care. I understand that Transformational Breath® makes no claim to benefit any specific condition and gives no guarantee as to outcome. I agree to keep confidential any comments, shares, or experiences by other participants at the events.

I understand that if I have medical concerns, it is my responsibility to consult a medical professional before participating. I will make the event facilitators aware of any medical conditions or physical limitations before each event. If I am pregnant, trying to become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate.

I understand that Transformational Breath® involves hands-on bodywork in order to promote a healthy breathing pattern. I consent to physical touch during the breath sessions and if I choose not to be touched during any session, I will explicitly tell my facilitators prior to the session and at any other time as necessary, that I do not want to be touched.

I, intending to be legally bound, hereby agree at all times to release, indemnify and hold harmless blosm studio, Transformative Sound LLC, The Transformational Breath Foundation (TBF), and the event leaders and facilitators, as well as their respective directors, officers, employees, agents, and beneficiaries, from and against any and all claims, causes of action, damages, liabilities, costs, and expenses, including legal fees and expenses, arising out of or related to participation in Transformational Breath® activities. I knowingly and freely assume all risks, both known and unknown, and assume full responsibility for my participation in Transformational Breath® activities.
